SSM Health Dean Medical Group, a 450+ physician multi-specialty group, is actively recruiting a full-time (1.0 FTE) Palliative Care Advanced Practitioner to join our team at our Janesville clinic.
Under the direction of the Division Chief, Palliative Medicine, the Advanced Practitioner – Palliative Care functions in an Advanced Practice role, the WI region Palliative Care Program providers co-manage patients with the referring specialists, they provide symptom management, discuss goals of care, promote quality of life, encourage coordination across multi-specialties, and facilitate complex patient and family decision-making. They will develop, implement, and evaluate care plans for adult patients with multiple or complex healthcare care needs in primary, acute, and long-term care settings, including the home.
The AP provides clinical expertise and mentorship to care team members throughout the continuum to enhance and inform palliative care delivery, serving as liaison to ensure continuity of care, communication and documentation.

Based in Madison, the Advanced Practitioner – Palliative Care, in collaboration with other Palliative Care Department providers, develops interdisciplinary programs, clinical practice guidelines, and educational materials to improve care for patients with palliative medicine needs throughout the SSM-WI Region.

The new medical campus in Janesville (www.stmarysjanesville.com) opened on January 9, 2012. It includes the 50-bed St. Mary’s Janesville Hospital and the adjacent Dean Clinic office complex. The hospital provides seamless health care services from Urgent Care to ICU. If a patient requires tertiary care, a transfer to St. Mary’s Hospital in Madison is easily accessible. Madison is 35 miles north of Janesville. Electronic medical records are connected throughout the hospital, clinic, and the entire Dean system through Epic.
