Decisions by Wisconsin health care teams to connect patients with Diabetes Self-Management Education and Support (DSMES) services at key points on the care journey are critical as the number of people with this chronic condition continues to rise.

  • What is DSMES and why is this a critical component for patients?
  • How do patients, with type 2 diabetes, access DSMES services?
  • What role does the registered nurse have in enhancing the health of patients during their diabetes care journey?

The Wisconsin Nurses Association presents this synchronous online learning session to any registered nurse in Wisconsin. Join to learn more about:

  • Components and benefits of DSMES services
  • Key times to refer patients for DSMES services
  • Components of a DSMES services referral
  • Reimbursement for DSMES services
  • Locations of DSMES services
  • Role of the registered nurse in team-based care for patients with diabetes
  • Opportunities for establishing collaborative relationships to support the efforts of DSMES services

Submit your questions for the presenter on the registration form or at the end of the webinar.

***WANTED:  Wisconsin Registered Nurses to contribute to our research. We are interested in learning more about Registered Nurse knowledge change about the Diabetes Self-Management Education and Support (DSMES) services using a synchronous online learning session. Participation is completely voluntary and will not impact your ability to earn continuing nursing education credits for attending the session. Study participants are asked to complete a short pre- and post- session survey, estimated to take no more than 5 minutes to complete. No session components will differ for those electing to participate. If RNs decide to join the project, please know that you may change your mind at any point and then your responses would not be included as project data. If you have questions about any aspect of the project or your involvement, please let Dr. Janet Reilly, Principal Investigator know (920-284-9205 or reillyj@uwgb.edu). When you begin the session, you will be provided the opportunity to choose to take part in this MSN QI project by giving consent for participation. If you give consent, you will be asked to complete the pre-session survey before starting the session. Once the pre-session survey is complete, the synchronous online learning session will begin by clicking on the link. At the conclusion of the session, RNs will be asked to complete the post-session survey. If you choose to participate, the link will move RNs through the post-session survey and then to the session evaluation for the CNE certificate. If you choose not to participate in post-session survey, the session evaluation and option for CE credit will be next. Thank you for your consideration to participate.
